Davis Vision Network Plan

To receive coverage, you must visit a participating Davis Vision provider. An automated Provider Locate System, supplying you with a recorded list (or printed list upon request) of the participating Davis Vision providers nearest your home can be accessed by calling (800) 406-1656.

If you prefer not to create an account log-in, you may go to www.davisvision.com, select Member LogIn and simply enter 4481 in the Client Code field.

To receive services from a Davis Vision participating provider, simply contact a provider and schedule an appointment. You may wish to contact the Vision Care Processing Unit at (800) 406-1656 to ensure it has been at least 24 months since you last received services (12 months if you are age 61 and over or age 14 or younger) and that the provider still participates in the Davis Vision network. Your provider will contact Davis Vision for authorization.

Davis Vision Provider Network Plan Coverage

The Vision Health Plan includes a comprehensive eye examination, including dilation, as professionally indicated, and one pair of prescription eyeglasses from the ‘Davis Vision Designer or Premier Frame Collection’ at no cost.

As professionally indicated, UV (ultraviolet) protective coating and standard anti-reflective coating (ARC) are also provided at no cost to the patient.

You can obtain other optional lens types and coatings if you make the required co-payment. This co-payment must be paid directly to the provider. Please reference the Davis Vision Provider Network Plan: Services and Co-Payments Table for payment information.

In lieu of one pair of eyeglasses, Collection contact lenses are available for most prescriptions upon payment of a $50 co-payment.
