To receive coverage, you must visit a participating DavisVision provider. An automated Provider Locate System, supplying you with a recorded list (or printed list upon request) of the participating DavisVision providers nearest your home can be accessed by calling (800) 406-1656. You may also visit DavisVision.com for a complete list of participating providers. Currently, over 300 vision care offices located throughout Massachusetts and the surrounding area participate in the DavisVision Provider Network.
To receive services from a DavisVision participating provider, simply contact a provider and schedule an appointment. You may wish to contact the Vision Care Processing Unit at (800) 406-1656 to ensure it has been at least 24 months since you last received services (12 months if you are age 61 and over or age 14 or younger) and that the provider still participates in the DavisVision network. Your provider will contact DavisVision for authorization.
DavisVision Provider Network Plan Coverage
The Vision Health Plan includes a comprehensive eye examination, including dilation, as professionally indicated, and one pair of prescription eyeglasses from the ‘DavisVision Designer Frame Collection’ at no cost. The ‘DavisVision Designer Frame Collection’ includes over 220 stylish eyeglass frames. You may select a frame from the ‘DavisVision Premier Collection’ for a $25 co-payment.
As professionally indicated, UV (ultraviolet) protective coating and standard anti-reflective coating (ARC) are also provided at no cost to the patient.
You can obtain other optional lens types and coatings if you make the required co-payment. This co-payment must be paid directly to the provider. Please reference the DavisVision Provider Network Plan: Services and Co-Payments Table for payment information.
In lieu of one pair of eyeglasses, Collection contact lenses are available for most prescriptions upon payment of a $50 co-payment.
